With a quorum of Board members present, the January 27, 2025 meeting of the Canton Community Health Fund opened at 6:15pm.

I. First order of Business:  Q/A & Vote on prospective new board member: Ann Gorjanc.

Ann’s education, community involvement & career as a Physician’s Assistant were reviewed.

MOTION was made to invite Ann to join the Board as a member, by Sandy, seconded by Linda and approved unanimously by the Board. Welcome Ann!  

 

MEETING #1: ANNUAL MEETING 2024 REVIEW & 2025 OUTLOOK: (Sylvia)

 THE ANNUAL REPORT IS REQUIRED BY CCHF BYLAWS & IS A REVIEW OF THE PAST YEAR, HOW IT BUILDS UPON PREVIOUS YEARS AND AN OUTLOOK ON 2025 THAT IS DIRECTLY INFORMED BY DECISIONS MADE, OPPORTUNITIES TAKEN AND CHALLENGES MET WHEN WE ASKED THE STRATEGIC QUESTION: WHAT’S MORE IN 2024?

I’LL BEGIN, LIKE I BEGAN 2023’S ANNUAL REPORT BECAUSE IT’S TRUER THAN EVER, BY CONGRATULATING THIS BOARD FOR A VERY PRODUCTIVE & SUCCESSFUL & COURAGEOUS 2024. THAT LAST WORD IS THE MOST IMPORTANT, AGAIN, BECAUSE IT TAKES COURAGE TO CHANGE, TO LOOK FORWARD, TO TRY NEW THINGS, TO STAY PRESENT & INTENTIONAL & INVOLVED WHEN THINGS GET CHALLENGING — SEEING NOT JUST WHO WE WERE OR EVEN ARE, BUT WHO WE CAN BE – SPEAKING TRUTH & ALWAYS HONORING OUR LEGACY GIVEN TO US BY OUR FOUNDERS THE CANTON VISITING NURSES ASSN.

REVIEW & RESULTS

LET’S BEGIN WITH THE JANUARY/2024 STATEMENT of GOALS

1) Expand Awareness: Goal: 100 FB Followers

WE HAD 47/48 FOLLOWERS IN DEC/2023, OUR 1ST YEAR ON FACEBOOK AND WERE UP TO 120 AS OF DEC/2024. IMPORTANTLY, COMMUNITY PARTNERS, BOARD MEMBERS & OTHER PEOPLE ARE SHARING OUR STORIES.

2) Have a smooth & successful Awarding Season.

WE DEVELOPED A METHODOLOGY IN 2023 THAT CONTINUED TO WORK IN 2024; IT KEEPS THINGS FAIR BECAUSE IT’S BASED ON DATA, NOT JUST PERSONAL OPINIONS/PREFERENCES.  IN 2024 WE ADDED THE BOARD’S FOLLOW UP QUESTIONS TO APPLICANTS, AS ANOTHER DATA POINT, & THE GREATLY IMPROVED IMPACT REPORTS WHICH FURTHER ENHANCED OUR ABILITY TO MAKE MORE OBJECTIVE, FINANCIALLY RESPONSIVE & STRATEGIC DECISIONS.

 

3) No one says ‘I’ve never heard of you’ ever again!

WE’RE NOT INVISIBLE ANYMORE. IN 2022/2023, EVEN ORGANIZATIONS WHO WE HAD GRANTED FUNDS TO FOR YEARS, DIDN’T KNOW OUR PROPER NAME BECAUSE WE WERE JUST ANOTHER CHECK.

TODAY, COMMUNITY PARTNERS REGULARLY PROMOTE OUR PARTNERSHIP & ARE PROUD TO BE IN THIS NETWORK OF COMMUNITY LEADERS. THEY APPRECIATE THAT WE’RE INTERESTED & INVESTED, AND NOT JUST FINANCIALLY, IN THEM.

LOCAL MEDIA KNOWS WHO WE ARE & HAS BEEN VERY SUPPORTIVE OF OUR MISSION & OUR CONNECTGIONN TO THE COMMUNITY.

 

4)  Become very well known.

5) BE ABLE TO SAY IN 2025 …. & NOW WE THRIVE!

WE HAVE RE-BUILT THE CYBER & OPERATIONAL &. MARKETING INFRASTRUCTURE – THE FOUNDATION…. THE BUSINESS – AND CONTINUE TO FINE TUNE IT INFORMED BY OUR COMMUNITY’S PRIORITIES.

WE HAVE AN AMAZING BOARD AND KEEP ATTRACTING THAT TALENT WE NEED & THIS COMMUNITY DESERVES.

CCHF’S BRAND – The Promise of the Experience

IN 2024, WE HAD OUR 1ST STRATEGIC PLANNING WORKSHOP CREATING A PICTURE OF WHO WE ARE & JUST AS IMPORTANT WHO WE AREN’T.

IN 2023/24 WE REFOCUSED OUR INTENTIONS AS A FOUNDATION VS JUST A FUND, A CHECK BOOK. THE FUND IS THE COMMUNITY’S MONEY. AND ITS OUR CHARGE TO KEEP IT SAFE AND TO SPEND IT ON COMMUINTY NEEDS TO ENHANCE HEALTH & SAFETY.

BUT, THE FOUNDATION IS WHAT WE REBUILT IN 2023/2024. WITHOUT A STRONG FOUNDATION TO SUPPORT OUR WORK, THERE IS NO FUND. THE FOUNDATION IS THE SOLID BUILDING BLOCK FROM WHICH WE CAN LIFTS UP THIS COMMUNITY’S HEALTH & SAFETY. THAT WAS AN ORIGINAL INTENTION OF CVNA.

COMMUNITY PARTNERS, A PILOT FORMALIZED IN 2024 ARE A KEY PART OF THAT FOUNDATION CEMENTING THE BIG HOLISTIC COLLABORATIVE VIEW OF OUR COMMUNITY HEALTH & SAFETY.

WE INTRODUCED OUR NEW PAY IT FORWARD SCHOLARSHIP. SO NOW WE, ONCE AGAIN INSPIRED BY THE CVNA, HAD 2 SCHOLARSHIPS, BOTH ON GOOGLE, COVERING NOT JUST HIGH SCHOOL STUDENTS, BUT ANY ONE IN THE COMMUNITY PURSUING A CAREER IN HEALTHCARE OR PUBLIC SAFETY.

 

OPERATIONS

WE TOOK A FULL-DOCUMENT DEEP DIVE IN INTO ALL BYLAWS AND OUR CERT OF INCORPORATION SO THAT THEY MORE CLEARLY REFLECT THE WAY WE ARE DOING BUSINESS TODAY AND WILL IN 2025.

TECH

WE CONTINUED REBUILDING INFRASTRUCTURE, INCLUDING OUR WEBSITE & EMAIL ACCOUNT BECAUSE THE 2023 MODEL WAS NO LONGER SUPPORTED OR RECOGNIZED BY GOGGLE, MICROSOFT & OTHER PLATFORMS. A REALLY BIG DEAL.

WE UPDATED OUR COMMUNITY PARTNER GRANT APPLICATION & IMPACT REPORTS ALSO RECREATED ON GOOGLE.

 

MARKETING

IN 2024, OUR MARKETING INITIATIVES PROGRESSED FROM AWARENESS BUILDING, WITH 2023 SPOTLIGHT & STORY SHARING SERIES IN SOCIAL MEDIA & MONTHLY IN VALLEY PRESS, TO A MORE IN-DEPTH EXPLORATION OF PERSONAL IMPACT WITH 2024’S COMMUNITY VOICES IN SOCIAL MEDIA & VALLEY LIFE, WHERE INDIVIDUAL CLIENTS, IN SOME CASES, SHARED THEIR PERSONAL EXPERIENCES.

OUR COMMUNITY VOICES STORIES APPEARED NEARLY EVERY MONTH IN PRINT PRESS/VALLEY LIFE, STARTING MARCH/2024 IN VALLEY PRESS, THROUGH THE SUMMER, FALL OF 2024 AND ENDING NEXT MONTH WITH THE FEBRUARY/2025 STORY ON THE FD, JUST A MONTH BEFORE WE OPEN APPLICATION SEASON.

WE ALSO HAD A PRESENCE, ONCE AGAIN, IN TODAY MAGAZINE, WINNING 3 EXCELLENCE IN JOURNALISM AWARDS, FROM THE SOCIETY OF PROFESSIONAL JOURNALISTS, FOR OUR 2023 COVER STORY ON OUR LEGACY & ANOTHER FOLLOW UP PIECE ON WHAT’S NEW WITH CCHF SINCE THAT COVER STORY.

WE ENCOURAGED COLLABORATIONS BETWEEN COMMUNTY PARTNERS – FAVRAH/SUMMIT, SOCIAL SERVICES/HEALING MEALS ARE ALL TALKING TO EACH OTHER ABOUT HOW TO WORK TOGETHER IN 2025.

THROUGH BOARD VISITS, OUR COMMUNITY PARTNERS HAVE ALSO SHARING THEIR EXPERTISE SO WE UNDERSTAND THEIR OPERATIONS MORE WHICH ENHANCES OUR DECISOIN MAKING ON BEHALF OF THE COMMUNITY. IMPORTRANTLY, THEY SHARED WHAT THEY’RE SEEING FOR TRENDS IN THE COMMUNITY, VALUABLE MARKET RESEARCH. MORE DATA!

WE WELCOMED A NEW CCHF MARKETING OFFICER, LINDA IN 2024. SHE’S QUICKLY BECOME NOT ONLY AN INDISPENSIBLE BOARD MEMBER, BUT SHE RESOLVED OUR CONFOUNDING INSTAGRAM ISSUE AND TOOK OVER THE MANAGEMENT OF OUR SOCIAL MEDIA AD PILOT WHICH WOULD NOT HAVE HAPPENED OTHERWISE.

LINDA PROVIDED A REVIEW OF RESULTS WHICH INCLUDED:

  • 50% OF THE SOCIAL MEDIA AUDIENCE IS FROM CANTON. THE OTHER IS FROM BRISTOL, SIMSBURY, AVON & TORRINGTON
  • 83% OF THAT AUDIENCE ARE WOMEN; 17% ARE MEN. AGE RANGE: 35-55.
  • IN TERMS OF ‘REACHES & ENGAGEMENT’ SINCE THE SUMMER CCHF’S COMMUNITY VOICES STORIES OF OUR COMMUNITY PARTNERS MADE A SIGNIFICANT IMPACT, FOR EX: FIRE/EMS STORY HAD 6,200 VIEWS; FAVRAH’S STORY 6,000 VIEWS; HEALING MEALS 5,800 VIEWS & SPIRITHORSE 4,800 VIEWS. LINDA’S BOOSTS ALSO LED TO 1-2 DONATIONS & AN INCREASE OF OUR 100 FOLLOWERS GOAL TO 120 BY YEAR END. BRAVA!

 

VANGUARD UPDATE

WE CREATED THE FIRST BUDGET, IN AT LEAST 10 YEARS & SAW AN

INCREASE IN OUR FUND’S ROI OF 8%, FOR A TOTAL INCREASE IN THE CCHF FUNDS OF $60,228.69.

2025 OUTLOOK

SO, WE’VE GONE FROM THE TWO STRATEGIC QUESTIONS: ‘WHO WILL BE IN 2023? TO WHAT’S MORE IN 2024?’ TO THE EXCLAMATION POINT – ‘IN 2025 WE THRIVE!’

SIMIPLY PUT, WE’RE READY. WE’VE NEVER BEEN IN A STRONGER POSITION, OPERATIONALLY, MARKETPLACE, OR STRATEGICALLY.

WE’VE EARNED IT. SO LET’S GET TO WORK!

ANNUAL STRATEGY WORKSHOP

CCHF’s Annual Strategy Meeting opened at 7pm, on Monday January 27, 2025, with a quorum.

 

REMAINING ITEMS from 2024:

  • TREASURER’S REPORT (Kristin)

Kristin presented the 9/30/2024 to 12/31/2024 Financial Report to the Board. For 2024, there were total withdrawals of $85,000, for a return prior to withdrawals of 15.28%.

The year end budget was also presented with $1,450 in donations & actual expenses of $67,375 which was $15,430 over budget, with grants (+9,415 and Pay It Forward Scholarships +$9,000) accounting for all of the excess spending.

A MOTION WAS MADE BY DIANE TO ACCEPT THE TREASURER’S REPORT AS PRESENTED. SANDY SECONDED THE MOTION. MOTION WAS APPROVED UNANIMOUSLY BY THE BOARD.

  • COMMUNITY PARTNER ISSUE: HEALING MEALS COMMUNITY PROJECT

HMCP contacted Sylvia to request another extension on their Jan. 31, 2025 Impact Report extension as they’ve only spent $821 of their Community Partner $2,500 grant. They’ve been seeking out other Canton organizations who may need their services, but have not found any. Of note, they also shared that prior to receiving CCHF’s grant in June 2024, they had delivered 469 meals to Canton residents totally $3,283.

HMCP has been a professional & excellent Community Partner since 2023 & provides a unique & critical service to Canton residents/their families, facing a health crisis.

Our accountant has advised that all grants must be distributed by year end & that funding extensions of any kind are a Red Flag to the IRS.

By majority, the Board voted for the following decisions:

Decision #1: An additional extension of time was denied. And no future extensions will be offered to any Community Partner or Scholarship recipient.

  • This decision is consistent with CCHF’s Operational Values:
  • Attention to compliance with operational/administration to ensure our reputation as a credible granting nonprofit corporation.
  • Accountability to the community especially with regards to the money donated to us to facilitate our work.

Decision 2: HMCP will be permitted to use the $1,679 balance of their 2024 Community Partner grant to offset the costs to serve Canton residents, prior to receiving their June/2024 funding.

  • This decision is consistent with CCHF’s Partnership Values:
  • Generosity, Integrity, Respect, Service to people, Empathy, Community, Health, Wellness/Well-being, Support.
  • Accountability to the community especially with regards to the money donated to us to facilitate our work.

These decisions will not impact HMCP ability to apply for a 2025 grant. Given the unpredictable nature of who in Canton will be needing their services, HMCP will be encouraged to present a full picture of how they are serving Canton residents, including those who require their services prior to the 2025 April 30th deadline if they apply for future Community Partner grants.

ITEM 1: REVIEW CORE TENANTS (2024)

 

There are certain well-documented phases in the Life Cycle of any strategy that the Board discussed in detail at our 2024 strategy workshop. Phase 2 is the Growth Stage – typically the next 2-5 years after Phase1: Re-boot & Establishment Stage which was 2022/2024.

Before starting on the framework for Phase 2 there are some Basic Building Blocks & Core Tenants that underlie every strategy & decision we will make, that we agreed on in 2024:

  • Accountability to the community especially with regards to the money donated to us to facilitate our work.
  • Doing our Due Diligence by being aware of what the community needs (THANK YOU COMMUNITY PARTNERS) and how that’s all evolving.
  • Attention to compliance with operations/administration to ensure our reputation as a credible granting nonprofit corporation.

-VISION: (What we hope to achieve or become in the future)

  • Making the evolving lives of our community better. Empowering, Creating Value, Offering Accessible Funding. Being Entrepreneurial. Partnering with community leaders who want our success as much as we want theirs.
  • Being actively in touch with what the community needs. Being involved. Paying Attention. Listening is the only way to know how our vision can be realized.

The Board agreed that this Strategic framework is still valid, without any additions or deletions, for how we will manage CCHF’s Foundation, the business, today and into the future.

 

ITEM 2: 2025 GOALS – GROWTH STAGE OF THE STRATEGIC CYCLE (2025-2027)

 

STEP 1: (ESTABLISH) DEFEND BRAND IDENTITY/POSITION

While we are in a far less defensive posture than we were in 2022/2023 we still have to protect the CCHF brand so that other organizations can’t negatively impact it by confusing the community as to who Canton Community Health Fund is.

  • SERVICE MARKS:

A State of CT Service Mark protects copy that describes how CCHF presents itself. If our Mission, Tag Line & other identifying copy is not service marked, those items can be used by anyone else and THEY can service mark it, preventing us from using that copy in the future.

Currently we have ‘CCHF’ & the ‘red heart logo’ service marked until 2027. This was a defensive move in 2022, when a committee, based in Hartford, started using our name locally. There are four other copy items (each with a $50 fee to the ST of CT for a total of $200), that Sylvia has recommended service marking:

    1. CCHF’s MISSION: ‘CONTRIBUTING TO THE HEALTH, SAFETY & WELL-BEING OF THE CANTON COMMUNITY’
    2. CCHF’s TAG LINE: ‘BY, FOR, ABOUT CANTON’
    3. ‘COMMUNITY PARTNER GRANTS’
    4. ‘PAY IT FORWARD SCHOLARSHIP’

 VOTE: After discussion, the Board voted by majority to pursue Service Marks for all four items.

 

STEP 2: (CONTINUE TO) BUILD AWARENESS/BRAND PRESENCE BALANCE OUR WORK WITH DEMAND

 – Based on who our Community Partners & Scholars are in June.

 

STEP 3: SEEK OUT OPPORTUNITIES & STRENTHEN RELATIONSHIPS  

  • Based on who our Community Partners & Scholars are in June.
  • The Board agreed to seek out potential Community Partners, educational programming or sponsorship applicants who are serving a need that isn’t being addressed in our community, ex: annual substance education/programming?

 

STEP 4:  FIND NEW DISTRIBUTION CHANNELS FOR PROMOTION & FUNDING

  • 2024: Instagram for Alumni Stories; ‘Community Voices’ series; Social Media Advertising Pilot

LINDA MADE A MOTION TO FORMALIZE THE SOCIAL MEDIA ADVERTISING PILOT AS PART OUR MARKETING PLATFORM & STRATEGY. SANDY SECONDED THE MOTION & THE MOTION PASSED UNANIMOUSLY

 

STEP 5: BEGIN TO REAP FINANCIAL BENEFIT OF WORK/REPUTATION

Donor Letter: This project was moved to CCHF’s June or Sept Meeting.

  • This is a project which will include: either compiling an historic donor list from files or purchasing one; letter drafting; mailing; tracking/reporting; thank you notes. This letter will be part of CCHF’s overall Marketing Plan vs a one-time letter, strategically combining CCHF’s social media Followers, framed with stories we’ll tell in 2025.
    • Diane agreed to work with Sylvia to review & consolidate the past donor lists as a basic starting point.
  • Board members were encouraged to reach out to their social media networks, friends, family & others who can be inspired/encouraged to Follow us on social media or support us otherwise. Sandy has been very successful using her contacts to generate donations!

+2024 GOAL TO 2025: HOW MANY NEW FOLLOWERS?

The Board set a goal of 300 total Facebook Followers for 2025!

  • DECISIONS
    • DR DITERS LEGACY SCHOLARSHIP/DOLLARS FOR SCHOLARS (Deirdre)

Deirdre distributed & reviewed a background/proposal memo on the recent history of CCHF’s administration of the Dr Diters Scholarship & the opportunity to collaborate with Canton Dollars for Scholars starting in 2025. CCHF would retain the Dr Diters brand & funding under its strategic platform & DFS would manage all assessment, administration & distribution.

DFS expertise & experience managing multiple scholarships for both CHS students as well as Canton high school seniors attending school out of town, who in the past confused the qualifications for the Dr Diters & Pay It Forward Scholarships.

The Board discussed offering Dollars for Scholars three $1,000 Scholarships, with potential for two more $1,000 Scholarships for total 2025 budget of $5,000. Deirdre volunteered to be CCHF’s liaison to Dollars for Scholars.

KRISTIN MADE A MOTION TO COLLABIORATE WITH CANTON DOLLARS FOR SCHOLARS, BEGINNING IN 2025, WITH A BUDGET OF $3,000 FOR DR. DITERS SCHOLARSHIPS (3-$1,000 Scholarships), WITH AN ADDITIONAL $2,000 IN RESERVE. DIANE SECONDED THE MOTION & IT PASSED UNANIMOUSLY.

  • SUCCESSION PLANNING

In 2025, the CCHF Foundation (the business), is stronger than it’s been in well over a decade. So, we need to think about how to continue to support the infrastructure that underlies the important work that we’re doing. We need to plan.

  • As Board Members: Part of our role as Board members is to replace ourselves, when we retire, with another individual with the interest, talent & commitment to doing work this Board has been charged with from our Legacy.
  • Sylvia’s Current Role: Board Chair; Foundation President/Business/Ops Manager; Board Secretary.

Admin Support:

Nobody joins a Board to take Minutes, coordinate schedules etc. However, that Secretary/admin role is required by the State of CT to be a legit non-profit. However, it is not a productive use of Sylvia’s time, so, we need to hire someone:

            – Their Role:

– Minutes, Financials/QuickBooks

-Assist with scheduling Community Partner & other meetings

-Special projects

-$20/hour maximum, with 0-10 hours of work per week.

– A 1099 & agreement & references will be required.

-All Board members were requested to seek our potential candidates.

-The Board discussed & rejected current AI options as a temporary option.

We’re looking for a retired person, an adult not a student. They will not be a Voting Board member per our Bylaws.

After discussion, the Board voted by majority to approve hiring a Secretary/admin support for $20/hour for 0-10 hours per week for a 2025 budget total of $5,000.

Split the Roles of CCHF Chair & President of the Foundation:

Sylvia presented the idea of splitting the responsibilities of Board Chair & President of the Foundation. They’re both critically important roles, but a lot for one person to be responsible for.

  • Board Chair/Community Ambassador would reach out to potential organization/Town/business collaborators to increase CCHF awareness, connections, reputation
    • Potential Collaborations: NW Bank Foundation; Canton/Avon Chamber of Commerce; Canton Main Street
  • Run Meetings, set Agenda
  • President of the Foundation: Operations Administration, Tech, Support Marketing, Finance, manage Community Partners – Running the Biz
  • Sylvia agreed to mentor a future Chair for the June-2025/2026 season, then they would assume the position as they get comfortable.

This topic will be more fully discussed at the Board’s June Marketing Meeting which is also our election meeting.

ITEM # 3:  ADMINISTRATION

  • MINUTES: Approval of Nov.18, 2024 Minutes
  • Formalize with a MOTION the documentation of Dec 2nd unanimous decision (through email discussion) on reallocation of $6,000 Community Partner grant from Focus on Canton to Canton Senior/Social Services.

 

SANDY MADE A MOTION TO DOCUMENT & APPROVE THE REALLOCATION OF FUNDING FROM FOCUS ON CANTON TO CANTON SENIOR/SOCIAL SERVICES & TO APPROVE THE MINUTES OF THE NOV. 18, 2025 MEETING. DEIRDRE SECONDED THE MOTION & IT PASSED UNANIMOUSLY.

 

ITEM #5: NEXT STEPS/ASSIGNMENTS
